The Major Change Form allows you to add/drop a major, minor, or specialization, or change your advisor.

• FERPA Release - Students may choose to complete and submit a FERPA Privacy Release request to grant authorized individuals access to their records
• Registration Action - Decrease/increase credits through last day to add, department consent, overrides, section change, time conflict, change to/from S/U grading, etc.
• Undergraduate Request for Readmission - Undergraduate students who leave the University for at least one semester, not including the summer semester, are required to apply for readmission
